When I was a boy radiographer in the 70's, the nursing staff had a direct line to the ambulance service, a facility that they never abused AFAIK. However, one reorganisation later, all call for ambulances had to be channelled through an administrator with no clinical background or judgement, and yet was to be the arbiter of who needed transport. This placed additional load on the nurses, who were no longer able to make decisions about patient movements until confirmed (or denied) by a bureaucrat who was paid more than they were.

It's been going downhill ever since.
